Person centred care is about ensuring the people who use our services are at the centre of everything we do. The ultimate goal of person-centred care is to create partnerships among care home staff, people with dementia and their families, to enhance the quality of life and the quality of care of people with disease.
It is delivered when health and social care professionals work together with people, to tailor services to support what matters to them.
The four principles of person centered care
- Treat people with dignity,
compassion, and respect - Provide coordinated care,
support, and treatment - Offer personalised care,
support, and treatment - Enable people to recognise
and develop their strengths and
abilities for an independent and
fulfilling life.
People can often lose their independence when they go into care, it’s important to always bear in mind that everyone using care services has their own thoughts, feelings opinions and needs.
These measurement includes:
- Making sure people are
physically comfortable and
safe. - Considering people’s
preferences and chosen needs. - People’s values and putting
people at the centre of care. - Provide emotional support
involving friends and family. - Making sure people have
access to the correct care that
they need, when and wherever
they need it. - Ensuring people get all the
information they need, in an
accessible way for them, to
make decisions for their care
and support.
